Job Description

Position Purpose:

The Social Worker is a vital member of the interdisciplinary healthcare team. His/her primary contribution is to conduct psychosocial assessments, provide support, and allocate resources for patients and families experiencing high-risk social/medical conditions.

Case Managers screen admissions and refer high-risk patients to the Social Worker for intensive management. The Social Worker conducts a complete psychosocial assessment and functions collaboratively with the Case Manager to coordinate services and implement a safe, effective discharge plan (based upon the patient choice process).

Education:

Bachelor’s degree in Social Work required, master’s degree preferred in social work.

Experience:

Two years’ fulltime supervised experience in a hospital setting, is required.

Behavioral Health experience is helpful.

Ability to relate and work with chronically ill and disabled patients of all ages and with emotionally upset and at times hostile people within the facility.

Theoretical knowledge of the clinical processes and continuum of care.

Motivation for self direction.

Ability to demonstrate leadership skills.

Ability to related and work with various managed care representatives, defining the continuing need for medical/psychiatric treatment.

Ability to understand and integrate financial and medical needs of the patient.

Speak and read English fluently.

Must be emotionally mature and able to function effectively under stress.

Licenses/Certificates:

Current state licensure in social work by the Commonwealth of Pennsylvania, is preferred.
