Position Summary:
The Patient Observer is responsible for maintaining patient safety for his/her assigned patient and to provide a safe and comfortable environment for those patients at risk of harming themselves or others. This observer will be assigned to monitored observation assignments.
Key Reponsibilities:
• Review and verbalize all related policies
• Receive report from RN and handoff from previous observer
• Verbalize the reason for the monitored observation
• Continuously observe the patient/s by viewing:
Through a monitor
From door of patient room
Inside patient room
• Document appropriately on monitored observation screen (or paper during downtime) every 1 hour
• Use bed exit alarm, chair alarm for all fall risk patients
• Notify RN when there is a need for personal care and cleanliness
• Offer nutrition and fluids as directed by RN, be aware of diet restrictions and/or chocking risk
• Offer diversion activities as directed by RN
• Notify RN when there are restless indications for need to toilet
• Accompany to bathroom if approved by RN (eg. Patient ambulates well on his/her own) and leave door open to visualize patient at all times
• Notify RN if patient has complaints of any kind for example: pain or constipation
• Verbalize about reporting escalating behaviors/change in mental status to RN immediately
• Demonstrate appropriate communication techniques:
Introduce self, including title and role
Address patient by surname, until patient requests otherwise
Communicate in a clear, calm manner
• Demonstrate how and when two patient identifiers must be utilized
• Demonstrate proper use of the hospital bed: leave in low position, set and reset the bed exit alarm, brakes, call bell
• Change bed position under the direction of the RN (eg head of bed up or down)
Required Knowledge & Skills:
• Able to communicate verbally and in writing in the English language.
• Able to complete mathematical computations required to perform treatment protocols.
• Exhibit a caring compassionate manner
High School graduate
Experience in a hospital setting
CPR/BLS certification
Meditech - HIPPA paperwork and Meditech access will be obtained through the nurse manager.
