This position is subject to the COVID-19 Vaccination Policy for Steward Health Care Workforce. Employees in this position will be required to provide adequate proof of vaccination for COVID-19 by an FDA-approved vaccine prior to starting work. Steward will consider requests for exemptions from this vaccine requirement on the basis of medical condition or sincerely held religious belief in accordance with applicable Federal laws.


The Lead Case Manager is responsible for assisting with all the daily operations of the Case Management Department. The Lead assists the Department Director in employee supervision, management, and training; data

research and analysis; development and implementation of quality initiatives designed to support defined objectives; and team resource- creating and maintaining a collaborative process of assessment, planning, facilitation and advocacy

For options and services to meet an individual's patients health needs through the continuum of care by communicating available resources to promote quality cost-effective outcomes. The Lead will act as a staff Case Manager if needed.


Graduate of an accredited school of nursing.

3 years recent acute care nursing experience required. General medical /Surgical and/or Critical nursing preferred. Utilization and managed care experience desirable.

Licensed by the State of Texas as a registered Nurse(Case Manager Certification preferred.

Stroke Awareness Education must be completed within 6 months of hire.

Reference Steward Certification Grid for all required Certifications.
