Our Safe and Ready program consists of a rigorous [five-point] standard ensuring patient safety, confidence and convenience: Expanded hours will allow previously cancelled procedures to be scheduled as quickly as possible. Any COVID-19 related care takes place in designated areas away from other patients and their families. Emergency Departments are reorganized to be a safe place to treat all emergency patients. A stringent cleaning policy has been implemented throughout the hospital. A strictly controlled visitor and masking policy is required for patient safety. You can rest assured that we have made the necessary preparations to provide care in a safe, controlled and professional way.

Qualifications: Graduate of an accredited school of nursing. Massachusetts Registered Nurse licensure, BLS, BSN, and Wound certification. Demonstrated clinical competence in the specialty area of nursing practice preferred.

Physical Requirements: Exerts negligible up to 10 pounds of force constantly to move objects; at least 20 pounds of force frequently, and ability to move heavy patients with assistance. Frequently reaches (extending hands and arms in any direction), and handles (seizing, holding, grasping, turning, or working with hands). Occasionally stoops (bending the body downward and forward by bending the spine at the waist), and kneels (bending the legs at the knee to come to rest on knee or knees).

Reports to: Clinical Nurse Manager

Function: all Registered Nurse’s function delegated by Holy Family Hospital within scope of practice as delineated by the Commonwealth of Massachusetts, including but not limited to leadership, education, provision and supervision of direct patient care, and assisting in overseeing the care and services provided by professional and ancillary staff. Assists manager and director in promoting and advertising to physicians and other referring providers to the clinic.

Clinical Practice and Patient Care Delivery Management

Collaborates with healthcare team, including but not limited to Case Management to ensure optimal disposition plans for all patients.

Monitors and adjusts staffing based upon patient census and acuity.

Manages the flow of patients to and from the department maintaining a focus on patient needs and efficiency

Supports and helps implement evidence-based practice that remains current regarding standards of care established by the profession, regulatory & accreditation bodies and hospital policy and procedures

Facilitates multidisciplinary care to ensure the best possible outcome for patients

Delegates responsibilities to unit staff to ensure positive achievable patient outcomes.

Collaborates with Skin/Wound Management team to develop and evaluate interventions to promote and maintain skin integrity.

Attends monthly Skin Committee pressure injury prevention meetings

Serves as resource guide to acute care staff by performing consultations to assess skin/wound conditions and treatment measures.

Assists in product recommendations for hospital par levels in accordance with the Steward vendor contracts

Works directly with physician and support staff to assess, document and provide wound care, dressing changes and education in the outpatient setting

Arranges home care and community support for patients discharged in the Outpatient setting.
