Qualifications: ADA Certified Diabetes Educator (CDE) or 1,000 hours diabetes education experience and proof of current diabetes continuing education. Registered Dietitian; MA State Licensure; MNT Provider status; MS preferred
Looking for a certified educator to help with all aspects of living with diabetes. Teach patients how to use medical equipment that is new to them, such as glucose monitors and insulin pumps. Plan out any lifestyle changes they need to make to manage the condition, for example by creating meal plans.
Assesses knowledge and skills of outpatients with diabetes to formulate and implement personalized education plans; instructs individuals and groups of patients with diabetes using appropriate audiovisuals and educational resource materials
Provides medical nutrition therapy (MNT) to outpatients with diabetes as per physician referral in a timely manner eliciting diet and medical history, explaining rationale for diet and providing written support materials.
Communicates with Medical staff information pertinent to the nutritional management of their patients.
Coordinates local diabetes program Performance Improvement [PI] Program, including data collection, analysis of outcomes and completion of PI report to be presented at Hospital Wide PI Committee.
Develops & Maintains Policies and Procedures for local diabetes education program and monitors department compliance to protocols.
Coordinates the day-to-day operation of the local outpatient diabetes program to include staffing, participation in hospital-wide committees, supervision of diabetes educators and secretary(s)
Coordinates fiscal components of the local diabetes program to include preparation of yearly budget and monitoring of expenses and revenues.
