Minimum Education:

Social Worker at entry level must have a Master’s Degree from an accredited school of Social Work.

Minimum Experience:

Additional experience in a setting related to primary assignment.

Minimum skills/abilities:

Requires excellent organizational and communication skills in the English language, and preferably in a second language.

The ability to deal with a rapidly changing health care environment and requires excellent interdisciplinary team collaboration skills.

Ability to handle multiple tasks.

Evidence of continuing professional development.

Ability to identify trends and make connections between care patterns.

Ability to critically think.


LCSW, LICSW preferred


The assignment requires the worker to continue his/her professional education through readings, seminars, workshops, etc.

Special Qualifications:

Must work with patients and families who are in crisis.

Be exposed to patients with major trauma, infectious disease, and/or emotional upset.

The Social Worker in this position is a Discharge Planner, working autonomously with the collaboration of a nurse case manager. Past experience as a discharge planner is preferred, but not required. Availability weekdays, weekends and several holidays is required.
