Position Summary:
The pharmacy medication history technician, working under the supervision and instruction of a licensed pharmacist, performs procedures and duties relating to the service’s Scope of Care. This can include collecting patient’s medications histories, as well as serving operational pharmacy technician roles in the main pharmacy when needed including preparation of both sterile and non-sterile medication dosage forms if applicable, automated dispensing functions, special project assignments, and prioritizing work responsibilities. For successful job performance, the pharmacy technician should be flexible and show keen attention to detail, exhibit initiative and be willing to work as part of a team. In addition, this individual should be cooperative and willing to cross train to fill in for other team members as necessary. The technician must be able to provide services as appropriate (under the direct supervision of a pharmacist) to meet the needs of neonate, infant, pediatric, adolescent, adult and geriatric patient age groups served at Rockledge Regional Medical Center and comply with all state and federal regulations.

Increased Skills/functionalities/assignments performed by the Medication History Technician can include, but aren’t limited to:

  • Expected to train and mentor other Pharmacy Technicians on Medication History Technician duties.
  • Interviews patients to obtain or verify allergies and medication histories including prescribed medications, over-the-counter medications, and dietary and herbal supplements
  • Contacts retail pharmacies and physician offices to complete or verify medication histories
    Enters home medication lists into appropriate computer system
  • Demonstrates proficient skills in using computer systems and paper charts for data retrieval
    Works closely with emergency department staff to obtain information for admissions
    Provides healthcare team with completed admitted medication histories
  • Promptly identifies any medication history issues with pharmacist and help identify and facilitate correction of medication discrepancies
  • Unit Inspection Functions
  • Operational Pharmacy Technician functions (including automated dispensing cabinets, medication transport, medication credits, compounding)
  • Communicates effectively with other healthcare professionals for medication reconciliation problem resolution; includes communication with patients and physicians

Candidates for Medication History Technician will be trained/skilled in all these functionalities.

The team member will be expected to be cross-trained for provision of services at any of the facilities throughout the Steward Health Care System. It is vital that an individual in this position be capable of good communication skills.

Access to patient information may be needed to fulfill the responsibilities of this role.
Adherence to SHC confidentiality policies and practices is required, as well as compliance with continuous regulatory readiness.
Job duties of this position may require access to protected patient information (PHI). The team member will be accountable for appropriate use of the record and compliance with all confidentiality and security policy and procedures related to use, access, and disclosure of PHI.

Job may require entry into secured areas where medications may be stored to fulfill specific duties related to this position.

All Tech Cert Level II Requirements must be met including:

    • Must have demonstrated cross training competency in all areas of Pharmacy as documented on the Pharmacy skills checklist.
    • 797 Compliant (Completed documented 797 skills check list & passed media-filled testing).
    • Documented ability and evidence of conducting monthly unit inspections.
    • Documented completion of Pyxis minimum and maximum inventory standards on a quarterly basis.
    • No evidence of written or suspension level corrective action over the previous 12 months.
    • Documented 40 hours of preceptor training for new pharmacy staff or students.

This position reports to the Director of Pharmacy, Lead Pharmacy Technician, as well as other pharmacists who are supervising his/her work.

The pharmacy technician position requires high school graduation. This individual should possess basic math and typing skills and must possess attention to detail and the ability to read and follow written instructions. The pharmacy technician should possess the ability to complete minimal computer functions.

Medication History technicians must have obtained (national) Certified Pharmacy Technician status.
Minimum of 2 years of previous experience as a Pharmacy Technician I or II. Retail pharmacy experience is highly desirable, while institutional pharmacy experience may be beneficial. General institutional/retail/health care experience may be helpful in the training of a pharmacy technician. Preference will be given to trained retail pharmacy technicians, those individuals who have completed pharmacy technician training courses and/or those who have obtained (national) Certified Pharmacy Technician status.

Note: Florida Board of Pharmacy requires that all person ordering and handling medications be registered with the Board of Pharmacy/State of Florida as technicians within 90 days of hire.

The pharmacy technician may occasionally be required to lift, push or pull weights of at least 25 pounds (not necessarily from a level lift). Position requires constant lifting, bending, carrying, walking, and routine pushing or pulling of medication carts and c-lockers. Applicant should be able to stay on his/her feet all day, work above shoulder level, and prolonged look in one direction (up, down or to either side). Pharmacy technicians should possess keen visual and hearing acuity.

The pharmacy technician will work primarily in the emergency department setting, but a component of routine assignments is deliveries to other areas. There will be frequent interruptions and the ability to smoothly change focus is a necessity. The preparation and dispensing of medications related to life and death situations requires consistent accuracy and may result in a highly stressful work environment. The pharmacy technician will interact with other team members to cooperatively complete work assignments in an accurate and timely manner.

The pharmacy technician should be able/willing to work variable schedules. Consistent attendance is a necessity for optimum provision of patient care/departmental services. It is expected the employee will comply with the hospital's satisfactory attendance standard.
