Position Function: The Per Diem Social Worker is a clinician responsible for social work services for the Adult Inpatient Psychiatric patient population at Steward St. Elizabeth's Medical Center.
This position is for day shift Monday-Friday work with the opportunity to work weekends once fully trained. The per diem social worker will coordinate their schedule ahead of time with the Social Work Manager to fill in for full time social workers while out and/or to provide extra support to the social work team as needed.
This worker will be under the supervision of a supervisory level LICSW, but will function with a great deal of autonomy and with specialized expertise in the area of Psychiatry.
The social worker will also work a part of the interdisciplinary team caring for patients in inpatient psychiatry.
II. Job Relationships: The Social Worker reports to and is accountable to the Director of Psychiatric Services.
The Social Worker collaborates with all staff on the inpatient psychiatry units.
III. Authority: Provide social work services on unit(s), assisting patients and families to cope with plans and decisions that need to be made before, during and after hospitalization.
Supervise and train graduate level social work students and other students when indicated and approved by the Social Work Supervisor and when the MSW has met the requirements for Supervision of the respective Graduate School of Social Work.
IV. A. Responsibilities/Essential Functions:
1.) "Provides superior customer service to internal and external clients, customers, and patients as referenced in the Service Excellence Standards."
The Social Worker may:
Provide individual and group counseling to patients.
Support patient advocacy.
Provide diagnostic evaluations and psycho-social assessments.
Provide referrals and/or follow-up services.
Collaborate with clinical interdisciplinary teams.
Facilitate continuing care/discharge plans.
Provide crisis prevention/intervention.
Assist in psychiatric safety planning.
Train onboarding social workers and provide secondary supervision to graduate students.
The social worker will provide counseling and psychoeducation for patients, family, friends, and other loved ones in regards to mental health and other psychosocial stressors.
The social worker will maintain a supportive relationship to patient and family/loved ones throughout course of hospitalization.
The social worker will provide case management/discharge planning for patients. This will include finding resources, engaging community supports, and placing referrals for mental health and substance use treatment.
The social worker provides documentation for social work activity in patient's record on a regular and timely basis.
The social worker consults with members of the medical center team in regards to the psycho-social needs of patients. Participates in staff in-service.
Opportunities to lead or co-lead groups for patients.
Engages with collateral, outpatient providers, and other community supports.
Perform biopsychosocial assessments for all patients on caseload.
Patients who will be seen may include those with histories of mental illness, cognitive limitations, child abuse or neglect, domestic violence and substance abuse.
Crisis intervention and discharge planning.
B. Responsibilities/Non-Essential Functions: The Social Worker will assess the discharge needs of all patients admitted to Geri-Psych / Adult Psych Units.
V. Reporting Requirements: The social worker will report to and works closely with the Inpatient Psychiatric Social Work Manager.
VI. Accountability: Shall be accountable for maintaining the confidentiality and security of all patient-related and medical -staff related data and information.
Shall be accountable for abiding by all relevant hospital polices and procedures.
VII. Qualifications:
Master’s Degree from an accredited school of social work, clinical or counseling psychology, or other human services related field and/or Licensed by the Commonwealth of Massachusetts to practice as a Licensed Clinical Social Worker (LCSW or LICSW), Licensed Mental Health Counselor (LMHC) or license in progress. Minimum of one year of experience providing direct treatment to clients with mental health or substance use disorders. Experience in a psychiatric inpatient setting preferred. Knowledge of DSM V preferred.
