POSITION: Nursing Home Liaison/Administrative Assistant

:

The NH Liaison/Administrative Assistant will be responsible for the management of office activities and daily operations for the Care Coordination, Social Work and Interpreter Services Departments. Management of the daily intake of referrals from Case Managers and Social Workers for patients requiring skilled nursing facility placement will be an integral part of this position. Communication to facilities and information on bed availability will be key functions to this role. This individual will also assist in data collection, statistical reporting, and project development for the Care Coordination, Social Work and Interpreter Services Departments. Scope of the role will include research, data collection, and reporting as required. This individual will provide support to the Care Coordinators, Social Workers and Interpreters as necessary and assist with timely facilitation of discharges as well as a timely response to patients’ requests for social work services and patients’ interpretation needs.

QUALIFICATIONS:

  • EDUCATIONAL:
    • Bachelor’s degree, Masters preferred
  • EXPERIENCE:
    • Office Management
    • Skilled Nursing Facility referrals
    • Data collection/analysis
    • Statistical Reporting
    • Medical processes and terminology
    • Project development a plus
    • Computer proficiency
