Nurse Practitioner, Valve Clinic, Full Time, Flexible Shift
"Serves as the day to day lead for outpatient and in-hospital coordination of the Transcatheter heart valve (Trans catheter Aortic Valve Replacement (TAVR), Mitral Clip, etc...) program. Fulfills an essential role in clinical navigation and patient education involving all aspects of care.
Works closely with multidisciplinary team including interventional cardiologists, cardiac surgeons, cardiologists, radiologists and other members of the health care team. Relationships with referring physicians.
Collaborates with the Valve Clinic Steering Committee to plan, develop and implement the Valve Clinic Heart Program. Works closely with cardiac surgeons, interventional cardiologists, cardiologists, fellows and other members of the health care team in providing care to the valve clinic patients. Coordinates patient access and medical assistance, schedules physician visits, diagnostic studies and structural heart procedures. Assists in implementing patient care for the Valve Clinic/TAVR program. Keeps detailed record of all patients referred to valve clinic including valve clinic tracking documents, consultations, workup, follow up etc. Manages close navigation of Valve clinic patients from the point of referral to post valve placement follow up. Able to understand, verbalize, and present clinical data such as patient history of present illness, past medical history, echo reports, cardiac catheterization reports, CT scan reports and the stage of navigation of each patients awaiting valve intervention to the cardiologists and cardiac surgeons. This candidate will closely work with the valve company to accomplish the necessary steps needed to get the patients ready for the valve intervention. Works with the hospital medical staff office to comply with appropriate documentations for proctored physicians. •
Works with community physicians and hospital admitting department to ensure insurance pre-authorization.
• Able to keep accurate minutes of the valve clinic meeting. Works with the Department Director, Medical Director of the Valve Clinic and the Medical Director of the Cardiac Surgery to finalize the cases, Agenda and Minutes to be presented at the valve clinic meetings.
• Collaborates with Marketing department to promote the structural heart program, by visiting physicians in the community and assist in community based health promotion programs.
Position Qualification: (Training, Education, Experience, Skills and Abilities)
"Excellent computer skills and familiarity with computer systems, electronic health records and data base management required. Clinical knowledge in Cardiac pathophysiology Exceptional written and oral communication skills. Minimum of 3 years of experience in cardiovascular care desired.
