Position Purpose: This position is responsible for the ongoing development and implementation of the Steward Health Choice Arizona (SHCA) Disease Management Programs, which aim to improve the health of SHCA’s members with Serious Mental Illness (SMI) and chronic health conditions, by intervening at the population, provider and member level. The Population Care Lead is focused on supporting the integration of physical and behavioral healthcare in Northern Arizona. Position duties include the following:

  • Analyze member utilization data to identify the most at-risk, highest need and highest cost members for referral to SHCA Care Management or Disease Management Programs
  • Develop and distribute member educational materials and provider resource toolkits for each chronic condition in the Disease Management Program annual calendar
  • Develop and deliver training sessions to providers on topics related to common chronic health conditions and the integration of physical and behavioral healthcare
  • Write articles regarding health promotion and integrated care for the SHCA member and provider newsletters
  • Manage a population-based caseload (such as members with HIV, members Hepatitis C on antivirals, members with chronic benzo/opioid utilization, or members with an alert for benzo/opioid overdose risk), as assigned by supervisor
  • Participate in provider site visits and committee meetings to promote the work of SHCA Disease Management Programs
  • Develop new interventions at the system level to address emerging trends and needs of the member population
  • Contribute to the analysis of program enrollment and outcome data for annual program evaluations and reporting to regulatory agencies
  • Collaborate with other SHCA departments to ensure data reporting and predictive analytics capacity is adequate to be able to appropriately monitor population health
  • Other duties as assigned by supervisor

Education / Experience / Other Requirements:


  • Current Arizona registered nurse (RN) license; OR
  • Master’s degree in a Behavioral Health Field with a current independent Arizona License in Social Work, Counseling, Marriage and Family Counseling or Substance Abuse Counseling
  • Preference will be given to candidates with an RN license.

Years of Experience:

  • Minimum: 2 years of experience
  • Preferred: 5 years of experience

Specialized Knowledge:

  • Knowledge of the AHCCCS Medicaid system
  • Knowledge of the principles and benefits of integrated health care and population health, or willingness to learn
  • Knowledge of common chronic health conditions and their treatment
  • Experience managing complex and unique caseloads for persons with both physical and behavioral health needs
  • Ability to document activities accurately and timely in an electronic medical record system
  • Ability to use data analysis tools to accurately map disease management process and to develop member registries for conditions
  • Ability to use and manipulate large volumes of member data
  • Ability to write analytic reports integrating multiple data elements and perspectives
  • Knowledge of Microsoft Office applications, especially Excel
  • Ability to prioritize and effectively handle multiple projects
  • Ability to work effectively in a team model
  • Ability to maintain confidentiality, privacy and security of protected health information and proprietary information
  • Ability to communicate professionally in written and oral formats
  • Ability to deliver presentations to large groups
  • Some travel required
  • Valid Arizona drivers' license
