Location: Rockledge Regional Medical Center

Coordinates entrance into outpatient cardiac/pulmonary rehabilitation providing smooth transition from other services. Implements the outpatient cardiac/pulmonary rehabilitation process an exercise plan, educational and psychosocial support. Acts as a liaison among physicians, staff, and community to provide optimal rehabilitative care to cardiac/pulmonary participants and their families. Helps develop individual plan of care for all clients. Collects data for discharge statistics that is reported to quality.

Licensure/Certification/Education (Required):

  1. American Red Cross OR American Heart Association Advanced Cardiac Life Support (ACLS)
  2. American Red Cross OR American Heart Association Basic Life Support CPR (BLS)
  3. Current/Active Florida Registered Nurse License (RN) OR Approved Multi-State License

Education 3: Technical School/2 Years College/Associates Degree

Licensure/Certification/Education (Preferred):
List any license, certification or education preferred for this job (e.g. driver's license, RN License, HHA Certification, Master's Degree, etc.)

Education 4: Bachelor's Degree

Specialized Skills (Required):

  1. Cardiopulmonary disease management
  2. CSU/Telemetry
  3. Knowledge of Nursing Process
  4. Strong cardiopulmonary monitoring skills

Strong interpersonal skills

  1. Knowledge of cardiopulmonary rehabilitation outcomes management

Knowledge of performance improvement process
