Registered Nurse plans and coordinates, and individualizes care for residents while maintaining standards for professional nursing practice. The registered Nurse supports the mission, policies and procesures of shron Regional health System/Transitional Care Unit and collaborates with all disciplines to achieve quality resident care.
Directs resident care based on the nursing process. Actively includes the resident, family, and other disciplines in formulating the plan of care including goals, physical, spiritual, and emotional needs.
Delegates appropriate aspects of care daily to other nursing personnel after assessing the resident. Responds to residents needs in a timely manner. Coordinates and documents health teaching for residents and families and documents residents and / or family response. Always provides resisdents and families with a complete explanation of procedures or treatments to be performed in order to reduce resident anxiety and promote cooperation and compliance. Consistently evaluates and documents the residents response to nursing care and treatments to be performed in order to reduce resident anxiety and promote cooperation and compliance. Consistently evaluates and documents the residents response to nursing care and treatments. Documents pertinent data relevant to the plan of care. Develops a therapeutic ans supportive relationship with the resident and maintains a proper prospective between personal and professional affairs
