The Social Worker must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients ranging in age from adolescence, adulthood and geriatrics. The Social Worker must possess the ability to assess data reflective of the patient's age, and interpret the appropriate information required to identify each patient's requirements relative to his/her age-specific needs.

Responsibilities & Standards

1.0 Provides crisis intervention for patients in need of immediate social work assessment according to Social Work Department guidelines within 24 hours of referral, assuming a reasonable caseload size, as periodically monitored and interpreted by Supervisor.

Provides psycho-social assessment, treatment and/or disposition, referring to outside agencies as indicated according to Social Work Department guidelines within 24 hours of the time of referral, with no more than one formal verbal counseling around non compliance per year.

Documents assessment and plan in medical record within 24 hours of the time of referral, with no more than one formal verbal counseling around non compliance per year.

Collaborates with medical, nursing and other personnel as needed per patient with no more than one to two legitimate complaints (as determined by supervisor) per year.

2.0 Provides concrete (e.g. clothing, housing) services for patients and/or families as needed and as soon as possible, assuming a reasonable caseload size, as determined by Supervisor.

Provides brief assessment, service and/or information/referral per patient according to Department policy with no more than one formal verbal counseling around non compliance per year.

Documents service provided per case in medical record, with no more than one formal verbal counseling around non compliance per year.

Collaborates with other hospital personnel and outside agencies as needed per patient with no more than one to two legitimate complaints, as determined by supervisor, per year.

3.0 Provides ongoing treatment to a maximum number of four patients per week.

Provides psycho-social assessment and treatment plan per patient according to Social Work Department guidelines within 4 to 5 sessions, with no more than one formal verbal counseling around non compliance per year.

Collaborates with appropriate in-hospital personnel or outside agencies according to Department Policy or per supervisor's request for each case, with no more than one to two legitimate complaints, as determined by Supervisor, per year.

Documents assessment, plan and progress in medical record per each contact according to Department policy, with no more than one formal verbal counseling per year.

4.0 Provides group treatment and leadership for weekly senior women's support group.

Provides group treatment and leadership for weekly senior citizen women's support group according to Department and N.A.S.W. guidelines so that there are no more than one to two legitimate complaints from the group as determined by Supervisor, in a one year period.

Collaborates with hospital personnel or family members according to Department policy as needed per patient with no more than one to two legitimate staff, patient or family complaints, as determined by Supervisor, per year.

Maintains progress notes after each group session, with no more than one formal verbal counseling around non compliance per year.

5.0 Attends meetings as requested and according to Social Work Department policy, unless excused or directed by Supervisor or Director of Department.

Attends scheduled Social Work Department meetings, and takes minutes as needed on a rotating basis, with no more than one formal verbal counseling around non compliance per year.

Selects and presents a case for case conference, as directed by Supervisor. The case presentation should reflect adequate preparation and analysis of psychosocial history and case dynamics.

Attends Supervisory meetings on a weekly basis to review cases. The Social Worker should be prepared to discuss and ask questions about case history, dynamics, treatment plan and goals, group activities, and caseload management issues. There should be no more than one formal verbal counseling around non compliance per year.

Attends Social Work committee meetings. Completes tasks assigned by the committee chairperson, with no more than one legitimate compliant, as determined by Supervisor, per year.

6.0 Completes statistical assignments according to department policy, on a monthly basis and within the time frame designated by Department Director.

Completes intake sheets monthly according to number of new patients seen 100% of the time.

Summarizes departmental activities -- e.g. conferences, staff meetings -- 100% of the time.

Completes case load carry-over sheets monthly 100% of the time.

Compiles data for Quality Assurance monitors in accordance with departmental, Hospital and JCAH standards as directed by supervisor 100% of the time. Presents same to departmental meetings.

7.0 Develops orientation program for new staff as requested, according to social work department policy.

Provides orientation regarding social work services as requested for emergency room and ambulatory care center patients for new staff -- i.e., emergency room, social work interns, and social work staff, with no more than one to two legitimate complaints regarding completion of this responsibility per year, as determined by Supervisor

8.0 Maintains current knowledge of social work theory practice and available community resources, as periodically monitored by Supervisor.

Attends sufficient continuing education programs to meet the Commonwealth of Massachusetts' licensing requirement of 20 CEU's per two year period.

Responds to questions from Social Work Department and Hospital Wide Staff regarding Community Resources, with no more than one to two legitimate staff complaints, regarding the quality of the interaction and/or content per year, as determined by Supervisor.
