Summary:

Responsible to Nursing Services for supporting the nursing philosophy and providing high quality, comprehensive patient care.⯠Responsible to the customer for courteous, accurate, timely service.

REQUIREMENTS:

  • Bachelor’s degree in social work required.

  • Current Ohio license as a Licensed Social Worker by Counselor Social Work Board required.

RESPONSIBILITIES AND MAJOR DUTIES

  1. Collaborates with interdisciplinary team to provide continuity of services.

  1. Appropriately assesses patient needs, including psychosocial, financial, post-acute, supportive, potential abuse, spiritual, and ethnic needs if applicable.

  1. Develops plan and coordinates post-hospital needs in a timely manner.

  1. Maintains social service records and related documentation.

  1. Demonstrates knowledge of Performance Improvement.

  1. Serves as educational resource and maintains current knowledge.

  1. Performs other duties as assigned.
