Supervisor, Performance Liaison
Job Description
Position Purpose: The Performance Liaison works in collaboration with physician practices to address opportunities to improve quality performance, clinical documentation, and patient experience
Support: Works in collaboration with provider offices to target opportunities for improved quality, clinical documentation, and patient experience. Maintains collaborative team relationships with peers and colleagues in order to achieve team goals, and to help foster a positive work environment. Evaluates provider and office staff workflow to identify opportunities for workflow improvement and is responsible for developing and implementing process improvement to enhance provider and staff workflow. Completes medical record data review/collection for appeals and exclusions at the time of data submission per contractual obligations. Ensures compliance with all applicable Federal, State and/or County laws and regulations related to coding and documentation guidelines for Risk Adjustment and HEDIS measures. Quality measure and clinical documentation subject matter resource for practices and Regional Team.
Performance: Interacts with assigned providers and office staff on a regular cadence to implement best practices and workflows to improve billing, clinical documentation, and resolve conflicting/ambiguous or non-specific documentation. Responsible for monitoring provider performance against network goals and creating a performance improvement plan to ensure all annual performance goals are met by each assigned practice. Escalate any issues at the practice-level in a timely manner. Monitor quality measures and evaluate strengths and weaknesses to address opportunities for improvement. Works collaboratively with Regional Team, Practice Staff, vendors, and SHCN colleagues to coordinate improvement efforts related to overall performance. Implements supervisor approved performance improvement plan at the practice level.
Reporting: Produces and maintains records. Keeps documentation up to date and reports progress to supervisor on a weekly basis. Provides dashboard reports to providers and practices and recommendations on improvement.
Coaching: Educates providers, practice staff and Regional Team on quality measure and clinical documentation required to meet regional performance goals. Develops and delivers training materials (Provider and Practice Facing). Provides performance training, on-site collaboration and on-going performance reporting.
Education:
- Medical Assistant or LPN license preferred, or equivalent clinical experience
Work Related Experience:
- 2-3 years of physician practice experience preferred
Specialized Knowledge:
- Knowledge of quality metrics and risk adjustment preferred
